Reporting Safety or Quality Issues
The VA Illiana Health Care System (VAIHCS) is committed to providing Veterans with high quality, safe patient care. To achieve safe patient care, we seek to provide care that both prevents errors and learns from errors when they occur. Such a system is based on a pledge by all stakeholders to a culture of safety. In order to achieve this goal, VAIHCS stives to maintain a ‘no blame’ culture.
