Program Information

The goal of this program is to support you in transition from active duty to civilian life.  We offer transition/readjustment assistance, care management and outreach services to Veterans.
Our program can help you receive the health care services you may want or need, along with providing the following services to you and your family.
 •Readjustment to civilian life
 •Individualized care, specific to your unique post-combat readjustment needs
 •Professional and confidential individual, marital/couple, and group counseling
 •Assistance scheduling and accessing any VA Medical Center services
 •Provide information and assistance regarding health care benefits and eligibility
 •Information and assistance accessing community resources
 •Education and information regarding VA benefits for which you may be eligible

Located in Building 98 of the Danville Medical Center.  Business hours are Monday-Friday 7:30am-4:30pm
We also have OEF/OIF/OND staff located at our Bob Michel Outpatient Clinic in Peoria, IL.
